If Keith had the kind of liver problems you're talking about, he wouldn't have a yellow hue to his skin, but he would certainly be retaining water in his legs and feet, which he obviously isn't.
He's had that belly for years. When they tour, he seems to work it off, but when they're between tours, he develops a paunch. As was pointed out by others, it's very common for men over 40 (not to mention a man in his sixties) to get a bit of a pot belly.
What makes me wonder is he's gone on record as saying he's had Hepatitis and cured himself. I'd love to know how he managed that.
I remember reading that article in a magazine, GTRM, and at the time I got the impression that Keith was saying all his doctors were scratching their heads not understanding how the hepatitis could have just vanished, but some people are blessed with immune-systems capable of totally eradicating a virus, I remember reading about the prostitutes in Africa who, although testing positive to the exposure of HIV virus, didn't have any active virus anywhere in their bodies and were perfectly healthy although people sleeping with them were dropping like flies, and HIV is a FAR more sophisticated virus than Hepatitis. It's just one of those things that happens in the long-term evolution of viruses and humans (host versus "parasite" warfare), there's always a few humans who will develop the ability to escape a lethal virus, it's just long-term biological adaptation.
